When logged in to your patient portal, make sure you click on the “My Health” tab on top, and the “Contact info” tab on the left side and review all relevant contact information is complete and accurate.

 

Next please click on the “insurance and Pharmacy tab” on the left to add a Surescripts Pharmacy

 

 

 

The next step will be to click on the “Medications” tab on the left side and enter all medications you are taking, or recently been taking. This list will populate as you enter a medication name. If you are unsure of the dosage or concentration, you can select the “add ____________ with unspecified dispensable” link on the top. If there are no medications click the “Mark No medications button” and press save and continue.

 

 

Once completed adding all medications, make sure you click on save and continue at the bottom to progress to the next menu.

 

 

 

 

As you click on save and continue you will be taken to the allergies tab and add any medication allergies. If you have no allergies, simply click the “Mark no known allergies” button and click on save and continue. As you enter the medication allergies, the list will auto populate. From the drop down add the allergies and click on the reactions and the severity from the list. Once complete click “save and continue” to move on.

 

 

 

The next tab is “Past Medical History”; click on all applicable diagnosis or click on the none box and select the past surgeries or click none and click the “save and continue” to move on.

 

For the ocular history, check all that apply and dates of diagnosis or additional details in the box that opens beneath the selected history and the same for ocular surgery. Once completed, click on “save and continue”. Complete the same information for social history.

 

Please skip “Quality Measures” “Implantable Devices” and the “Problem List” tab.

 

The next tab is the family history tab, and will auto-populate as information is added, please click the boxes on the family member affected by the condition.

Once complete, please click save and log off. You are now ready for your exam visit!
